Appointment Requests

Submission of a service request does not guarantee appointment availability. All appointments must be reviewed and confirmed by Aeris Phlebotomy Services prior to scheduling.

Patient Responsibilities

Specimen Collection Limitations

Aeris Phlebotomy Services will make reasonable efforts to obtain the requested specimen. However, successful specimen collection cannot be guaranteed in all circumstances.

Certain collections may be unsuccessful due to factors beyond the control of Aeris Phlebotomy Services, including difficult venous access, patient condition, laboratory requirements, specimen quality concerns, or other unforeseen circumstances.

Limitation of Liability

Aeris Phlebotomy Services is responsible only for the specimen collection process.

We are not responsible for:

  • Laboratory testing procedures

  • Laboratory test results

  • Medical interpretation of results

  • Decisions made by healthcare providers based on laboratory results

  • Laboratory delays

  • Lost specimens after transfer to a laboratory or shipping carrier

  • Rejected specimens due to laboratory requirements

  • Testing delays caused by third-party laboratories or carriers
