Services Provided
Aeris Phlebotomy Services provides professional mobile specimen collection services. These services may include the collection of blood specimens and other laboratory specimens for testing by designated laboratories or healthcare providers.
Aeris Phlebotomy Services does not perform laboratory testing, medical diagnosis, or medical treatment. Laboratory testing, analysis, and interpretation of results are performed by the applicable laboratory or healthcare provider.
Appointment Requests
Appointments may be requested through our website, phone, or email. All appointments are subject to availability and confirmation.
Submission of a service request does not guarantee appointment availability. All service requests must be reviewed and confirmed by Aeris Phlebotomy Services before an appointment is confirmed.
A service request does not constitute a confirmed appointment, and no payment is required at the service request stage unless otherwise communicated.
Patient Responsibilities
Patients are responsible for providing accurate and complete information when requesting services, including:
Correct contact information
Physician or healthcare provider orders when required
Accurate appointment and service details
Any required patient preparation according to the laboratory or healthcare provider's instructions
Aeris Phlebotomy Services provides the applicable laboratory or specialty test kit when a kit is required for the requested service. Patients are not responsible for purchasing or providing the test kit.
Patients are responsible for following all applicable laboratory and healthcare provider instructions, including fasting requirements, preparation requirements, and other test-specific instructions when applicable.
Failure to provide required documentation or complete required preparation may result in delays, an additional visit, or the inability to complete the requested service.
Payment
Applicable service fees, travel fees, and other charges will be communicated before confirming an appointment whenever possible.
Payment requirements may vary depending on the service requested. Payment may be required before, at the time of, or immediately following the appointment.
Laboratory testing fees are separate from Aeris Phlebotomy Services' service fees unless specifically stated otherwise.
Specimen Collection Limitations
Aeris Phlebotomy Services will make reasonable efforts to obtain the requested specimen. However, successful specimen collection cannot be guaranteed in all circumstances.
Certain collections may be unsuccessful due to factors beyond the control of Aeris Phlebotomy Services, including difficult venous access, patient circumstances, laboratory requirements, specimen quality concerns, or other unforeseen circumstances.
Aeris provides up to two venipuncture attempts per patient during a scheduled collection. If collection cannot be completed after two attempts, Aeris may recommend that the patient visit another laboratory or healthcare facility.
If a collection is unsuccessful after two attempts due to difficult venous access or circumstances beyond the patient's control, the patient may be eligible for a 50% partial refund of the specimen collection service fee, subject to review.
Travel fees, specialty kit fees, shipping or carrier drop-off fees, priority scheduling fees, optional add-on services, and other applicable fees are not included in any partial refund.
Limitation of Liability
Aeris Phlebotomy Services is responsible for the specimen collection services it provides.
Aeris Phlebotomy Services is not responsible for:
Laboratory testing procedures
Laboratory test results
Medical interpretation of laboratory results
Medical decisions made by healthcare providers based on laboratory results
Laboratory delays
Lost specimens after transfer to a laboratory or shipping carrier
Rejected specimens due to laboratory requirements or specimen quality concerns
Testing or delivery delays caused by third-party laboratories or shipping carriers
