By accessing this website or requesting services from Aeris Phlebotomy Services, you agree to the following terms and conditions.
Services Provided
Aeris Phlebotomy Services provides professional mobile specimen collection services. These services include the collection of blood specimens and other laboratory samples for testing by certified laboratories.
Aeris Phlebotomy Services does not perform laboratory testing, medical diagnosis, or medical treatment. Laboratory analysis and interpretation of results are performed by licensed healthcare providers or accredited laboratories.
Appointment Requests
Appointments may be requested through our website, phone, or email. All appointments are subject to availability.
Service fees may vary depending on:
Travel distance
Appointment time
Same-day or urgent requests
Weekend and holidays or after-hours appointments
All applicable service fees will be confirmed prior to scheduling.
Patient Responsibilities
Patients are responsible for providing accurate information when requesting services, including:
Correct contact information
Physician orders when required
Laboratory test kits or instructions when applicable
Failure to provide required documentation may result in delays or inability to perform specimen collection.
Payment
Payment requirements and service fees will be communicated prior to confirming an appointment.
Cancellation Policy
Appointments may be rescheduled without penalty if notice is provided at least 24 hours prior to the scheduled appointment time.
Cancellations or rescheduling requests made on the same day of the appointment will be subject to a $20 cancellation fee to cover scheduling and travel preparation.
If a patient does not provide notice and the appointment cannot be completed upon arrival, the appointment will be considered a no-show and the service fee will apply.
For patients who purchased laboratory testing directly through Aeris Phlebotomy Services, only the mobile specimen collection fee will be charged in the event of a same-day cancellation.
Travel fees may apply once the provider is en route to the appointment.
Aeris Phlebotomy Services reserves the right to require payment in advance for future appointments after repeated late cancellations or missed appointments.
Limitation of Liability
Aeris Phlebotomy Services is responsible only for the specimen collection process.
We are not responsible for:
Laboratory testing procedures
Laboratory test results
Medical interpretation of results
Decisions made by healthcare providers based on laboratory results

Travel Policy
Aeris Phlebotomy Services provides mobile specimen collection services throughout the Upstate of South Carolina.
Our mobile service includes travel to the patient’s location. Travel fees may apply depending on the distance from our primary service area.
Appointments within our primary service area may be included in the standard service fee. Appointments located outside this area may require an additional travel fee based on distance.
Travel fees will always be communicated and confirmed prior to scheduling the appointment..
Changes to These Terms
Aeris Phlebotomy Services reserves the right to update or modify these Terms of Service at any time. Any updates will be reflected on this page.
